Submission Process Summary: 

Submissions to ESCRO are made using Smartsheet web application forms.  The submission process consists of the following steps:

1. The investigator completes the appropriate form for the submission to ESCRO using the links below.  Investigators are advised to submit the application in one sitting, since the form cannot be revised within smartsheet once it has been submitted.    
2. Once submitted, the form will undergo review by the WCM ESCRO administrator, who will check the form for completeness and accuracy.  
3. If any issues with the form are identified, the WCM ESCRO administrator will return a tracked word file of the form in an email.  The file includes comments for the investigator to address. 
4. The investigator is required to respond to all comments in the form, keeping the tracked function in the document active.  Once all the items are addressed, the investigator will then re-submit the form to WCM ESCRO using the email WCM-ESCRO@med.cornell.edu.  
5. Steps 3 and 4 may be repeated if additional issues are noted by the WCM ESCRO administrator.  If all comments are addressed with no additional issues noted, the WCM ESCRO Administrator will route the application to the ESCRO Committee for review at the next meeting.  A copy of the final version of the application will be sent to the investigator for their regulatory records.

Notification Forms: 

New studies requiring ESCRO oversight must complete the applicable Notifiaction Forms below for each area of covered ESCRO research.  Please note that a single study may require the completion of more than one form.

Research involving hES and/or covered hPS cells 

Research designed or expected to generate human gametes

Research involving the use of human embryos for purposes other than the derivation of hES cells
